In a woody dicot, the apical meristems are formed during embryogeny. Cork cambium is the last formed meristem and it is completely secondary meristem in both stems and roots. Fascicular cambium is part of apical meristem of stem (Procambium) retained in vascular bundles and forms secondary vascular tissues of stem. In roots, vascular cambium is formed from both conjunctive tissue present below the phloem and pericycle present opposite to xylem.
