Thyroid-stimulating hormone (TSH), is a glycoprotein hormone produced by the anterior pituitary. Its primary role is to stimulate the thyroid gland to produce thyroid hormones. The release of TSH is governed by the hypothalamic-pituitary axis. Specifically, neurons in the hypothalamus release thyroid-releasing hormone (TRH), which stimulates the thyrotrophs in the anterior pituitary to secrete TSH. This, in turn, prompts thyroid follicular cells to release thyroid hormones in the form of T3 or T4. While T3 is the active hormone accounting for only 20% of the released hormone, most T3 originates from peripheral conversion of T4 to T3. Thyroxine (T4), making up over 80% of secreted hormones, is transformed into T3 through de-iodination upon entering circulation. T4 and T3 can then exert a negative feedback effect on the anterior pituitary: elevated T3/T4 levels decrease TSH secretion, whereas low T3/T4 levels increase it.
