The grafted kidney may be rejected in a patient due to a cell-mediated immune response. Graft rejection occurs when the recipient's immune system attacks the donated graft and begins destroying the transplanted tissue or organ. The immune response is usually triggered by the presence of the donor's own unique set of HLA proteins, which the recipient's immune system will identify as foreign. Transplant rejection is caused primarily by a cell-mediated immune response to HLA antigens expressed on donor antigen-presenting cells (APCs) transferred along with the transplanted organ. Recognition of donor HLA antigens on the cells of the graft induces vigorous T cell proliferation in the recipient. The T-lymphocytes mediate CMI (Cell-Mediated immunity).
